Online forms available:

1. Year 11 2025 Commitment Form -Students currently enrolled at JPC do not need to re-enrol for Year 11 2025. However, to assist us with our planning for next year, we ask that you complete the Commitment Form to indicate your intention for your son or daughter to continue their studies at JPC next year.
   
2. Leaver's Clearance Form - for students who are leaving JPC and moving to another school.
3. Car Registration for Senior Students at JPC - for JPC Senior Students who drive to and from school and park their car on campus.
4. Photo & Media Permission form for JPC Students - update student details
