The leaver's form is for any student that is leaving JPC.
The information required is mandated by the legislation administered by the Education Directorate ACT. JPC is required to update the student movement register with the Department of Education.
Note - School fees will be payable for the full term and final payment is to be made by the departure date. Please refer to the School Fees Policy - https://cg.catholic.edu.au/parents/policies/
